Student Health Service – Forms Available

Dear Parents,

The Student Health Service of the Department of Health is now enrolling students for the 2016-2017 school year. Forms are now available for pick up in the main office.

Please return all completed forms back to the main office by September 14 2016. After this date, you will need to fax completed forms directly to the Department of Health at 2915 1499.

Year 8 students are also eligible for a Hearing Screen Test. If you would like to more information about the Hearing Screen Test, please ask your child to see Roshni Mulchandani in the main office to pick up details.
