STUDENT HEALTH SERVICE – FORMS AVAILABLE

Dear Parents,

The Student Health Service of the Department of Health is now enrolling students for the 2017-2018 school year. Forms are now available for pick up in the main office.

Please return all completed forms back to the main office by Friday 14 September. After this date, you will need to fax completed forms directly to the Department of Health at 2915 1499.

Year 8 students are also eligible for a Hearing Screen Test. If you would like to more information about the Hearing Screen Test, please ask your child to see  Ms Mulchandani in the main office to pick up details.
